Heading 3 not appearing in table of contents

I have set the levels appropriately under 'Outline numbering', yet only Heading 1's and Heading 2's only appear under the table of contents.

Using LibreOffice 3.4.4 Any ideas?

Right-click Table of Contents and select Edit Index/Table. In the Index/Table tab set "Evaluate up to level" to a higher value.
