There is an additional line in the table of contents page that nothing to do with my document. I did a search through the document and the key words do not show up except for the table of contents. I tried cutting the contents of the page and it still shows up but on a different page sometimes. How do I remove this?

It doesn't show up there at all. At the bottom there is a drop down that has 4 options and I checked there as well.
