I want sign a document, when I click in "Sign Document..." button I watch a Window "Insert a smart card.", but my smart card is connected. I click the "Cancel" button twice and then I watch my digital certificates. Why is this happening? How can I fix this?

Per @manj_k, bug filed as:
fdo#54499 - Digital signature, "Insert a smart card", but my smart card connected

Current status: UNCONFIRMED

Perhaps we need someone with an actual smart card/smart card reader to try to repro. @Donald -- know anyone else who has the same hardware/software who can try to reproduce?

Alternatively: Can you still reproduce this issue when using the latest release ?
