Can I make a .pdf consent form that customers can fill in?

I use Libre office in a very basic way. But someone suggested that we need a .pdf form on our website that people could download, fill in and return to us to book events. Is there any way to do this (or something similar) in Libre Office?

Yes, with LibreOffice Writer you can create PDF forms that users can fill-in with a simple PDF viewer. How-to guide:

Yes, you can. Go to View > Toolbars > Form Controls.
