Create-forms in base: lefthand choices disappeared, need SUBMIT control

I installed Ubuntu 12.04 yesterday and began to try out LibreOffice BASE, using a pre-existing database object from OpenOffice.

At some stage, while I was creating a new FORM in BASE, I clicked something on the (leftish) bottom which caused the left-hand vertical menu of form-items to disappear.

[1] How do I get them back?

[2] When I have them back, how do I create a SUBMIT button (or the like) so that the new form will not merely accept the typing-in of data for a record but ALSO insert it into the associated TABLE?

open your form in EDIT mode.
Now, (Top Menu) View → Toolbars → Check the toolbar that was missing (I think it was Form Controls)

But I am thinking you might need a PUSH BUTTON (Form Controls will have it) and a macro.
