page renumbering in table of contents

I have created a master document that has a table of contents in it. I have then created some files that are for various chapters. The Preface, Chapter 1 and Chapter 2 number correctly. Chapters 3, 4 & 5 all restart numbering at #1 in the Table of Contents.

On the first page of each chapter the first paragraph is a Heading 1. It is set with Breaks to Insert = Page, Position = Before, With Page Style = Default, and Page Number = 0. All of these chapters are created from a template.

Any ideas as what is causing this?

This is most likely a page style or page break type issue. I usually require chapter-specific page, header, and footer styles which need to be defined in the template (or each sub-document) so that they can be imported into the master document. I would encourage creating a series of page styles (ch01, ch02, etc.) or some uniform page styles (Chapter text OR Left page and Right page) rather than use the Default page style.

When you click on the page break between ch02 and ch03 and select Edit Page Break… what do the settings show?
