is there a way to pull up an Avery address label template 15264

That one is not in LO. You can put in the specs as user defined or get a Word template from Avery (click here) read into LO & save as LO template.
I just did this. Worked out beautifully. Went on Avery site and clicked on create Word template for my label template #8160. Opened it in Libre written document, filled in my labels and saved. Then printed to the labels. Perfect. Thanks for the question and answer!
