How display images in OS X text document?

Images do not display in a Writer document.

It matters not whether they are inserted, pasted, or dragged into a paragraph or a table cell. I have read several messages regarding inability to display images. The solutions called for menu choices that don’t appear in my tools menu. My inserted or pasted images show up as a small icon with red, yellow, and blue rectangles. When I insert an image, the Preview box of the insert file dialog remains gray, inactive. A linked image adds some linking text. Memory for images is set at 20 MB. The hard drive has plenty of remaining space. How can I get images to display in the document?

Edit: I find that I can view the images in Print → Preview and that the images print without problem. I would still like to know if it is possible to view them in the document.

Which version of LO? There was a known problem with image handling early in the v5.0 series.

Using: OSX 10.10.* (Yosemite) with LO Version: 4.4.7.2

The application uses a writer template (*.ott file) which has an image in the footer. [It doubles as a push button which saves the document with an automatically incremented version number.]

Importantly there’s no problem with writer documents created from the template. The image for the template was created inside a text box.
