I use Mac 10.7 I created a form for client intake using writer; however when I save in .docx format and reopen the file the form controls are all gone. The only thing I’m left with is a skeleton; first name, last name, etc with no input boxes next to them. How do I fix this problem?

I’ve heard from multiple users that round-tripping isn’t working perfectly well with some of the OOXML file formats like DOCX and XLSX. For now, please save your files using the native file formats like ODT and ODS, and only use DOCX format for final export.

If you’re not running it yet, please install the latest release and check to see if you’re still having a problem with missing form controls.

Please file a bug about this problem and provide a list of steps that we can use to reproduce your results. It would be great if you could attach an ODT file showing some of the form controls you were using, and a DOCX export of that file showing which of the controls were missing or changed.
The QA team will be happy to help you track down this issue!
