I'm having real trouble getting a decent Table of Contents into a doc file.

I've set up the chapter headings and all seems fine here. The dialog that shows the structure is really non-intuitive imho and when it creates the TOC it seems really odd.

I cannot get it to indent the 1.1 entries but it's indenting the 1.1.1 entries. Also when the section number goes into double figures (so from section 10 onwards) the title of the section shoots over to the right hand side with a row of filler characters "." the the title on the far right then another row of filler characters and then the page number. Looks hideous frankly.

Yes, it is important to always allow enough space (indent or TAB) to cater for the number of digits in the leading numbering. Please remember (once you have enough karma) to mark your answer here as correct.

You have something strange going on with tab stops. Right click, Edit paragraph style, Tabs and set these to some sensible values.

I'm not sure I follow your advice, which paragraph style needs to change? I've gone through them all multiple times and I cannot see what the issue is. I've reduced the TOC to evaluate just one level and it produces this:

This is so strange. All the headings have the same paragraph style applied to them so why on earth does it produce such an odd TOC?

Check @mahfiaz's answer. You haven't given us enough context - what are the settings in the "ruler" area showing? See this example:
