In some documents all linked files are recognized. In others many are not and it appears to be arbitrary. Also the link it shows is incorrect. It adds an extra part to it, which is the link to the document itself, not the file linked into it. How can this be resolved?

By filing an appropriate bug report, which would inform developers about the issue; with relevant documents, which demonstrate the problem; and with correct description (including OS and LO version).
