If you have not yet assigned a title when creating the diagram, you can do so in the menu under Insert - Title. Before you can do this, you have to bring the diagram into edit mode by double-clicking on it. Once you have created a title, you can edit it by double-clicking on it, or you can use the Insert - Title command again.
