Why are my even page numbers missing in the footer? - the odd ones are good, and I have even worked out how to start the page numbers from the 5th physical page of the document (shame that working with pages is such a pain)

Page styles allow you to define if the headers/footers have the same content for odd and even pages: Format▸Page▸Footer tab▸Footer on▸Same content left/right (for the current style).
If the box is not checked (different content) must define the content of the header/footer for even pages and the odd pages. In your case, insert the “missing” page number field in the footer.
