Is there a sample / simple document template with index and REFERENCES /LINKS to indexed pages?
I found indexed template but it has no example how to link the index to the contents / page.
Should I be able to click on index and get to the linked part?
Currently (LO 5.3.0.3) there is no way to automatically create links from the index back to the content.
This is a a rather bad limitation in LibreOffice especially when creating long documents.
It can be done manually, but obviously that would be a long and painful process.
There is a Bug report / feature request that has been outstanding since 2013. !!!

You may want to add your request to that bug thread to see if we can get some movement on this.

Tip: download the ODT file and then Export to PDF with bookmarks.
Much easier to find things with bookmarks.
For some unknown reason the PDF file has no bookmarks.
