Adding Form Field to header area defaults to main document

I am trying to add a form field in the header area of a document.
I select a spot in the header,
Select Form design
Select add Text Box
And the Text box appears in the main document.
I am probably doing something incorrectly here, but not sure what it is.

Your form controls also just missed the header.
