Sheet insert columns columns before and after is greyed out. Right clicking a column doesn’t provide an the insert column either (like right clicking a row does). Track changes is not on. Surely there is existing help for this, but the LibreOffice search routines cannot find it if there is. HOW DO I INSERT A COLUMN?
Do you perhaps have a range of cells protected that might intersect with the area where you are trying to insert?

Never mind–evidently you must select a cell and cannot select a column. LibreOffice behavior and lack of online help always amazesme.
The problem looks specific to something on the concrete sheet, though, as @robleyd pointed out. Because on a clean sheet:
And anyway, there’s a menu, which allows fast inspection and finding the prominent elements under Sheet
top-level item. And the menu items are indeed discussed in the online help, and easily discoverable there using the insert column

But of course, the menu is such an unfriendly thing, that is hated to the point that every “modern” UI concept tries to remove it, to be more “user-friendly”. Well, even the tabbed variants have the insertion elements on the “Home” tab:
And when you are going to lock the sheet you are prompted if you need to insert/remove columns or rows on the protected sheet.
