I think that page has been created in a word processor like Writer.
I assume the orange gradient fill is Heading 1 (apply by pressing Ctrl+1), the Grey fill is Heading 2 (apply by pressing Ctrl+2) and the yellow highlighting means something so I have given a paragraph style name of Faults.
