Unwanted dialog box in a PDF Form generated from an ODS file

Why am I getting this unwanted dialog box in a PDF Form generated from an ODS file.
Its the Box with “@ Cancel and Post” at the bottom.
(see uploaded ODF with screen shot)

How can I prevent a user of the form from getting this msg box?

How I generate the PDF file (choices made)

File → Export as → Export as pdf…

Range – All

Images: jpeg

General:

• Create PDF form

• submit format – PDF

• Allow for duplicate names.

It is Adobe Reader bug adding a comment every time you press the Tab key.

In Adobe Reader click Menu > Disable new Acrobat Reader and allow it to restart. The Tab key should now work correctly.
NB some people have reported that you can then click View > Enable new Acrobat Reader and the Tab issue will not recur. Cheers, Al
