This is nothing more than an elaboration of this question → Help! I am a Newbie at databases. Can anyone give me a general idea(in layman’s terms) of what this error means?.

Also just noticed, you are not working with LibreOffice but rather OpenOffice. You may want to post here → AOO Forum
You can try here but people answering may not have AOO installed.
After more looking, it appears your form list box for Customer Type may not be set correctly. Cannot tell from images. Best to actually post a sample in the other question.
