I’ve set automatic captions and this works fine for the first 3 figures
but then the next one gets “Figure 1”. All settings are the same

When you added the fifth caption, did you use the exact same name for caption? Captions have different numbering based on their name.
@alobo – Did you verify the name, as @mahfiaz suggested?
If so, and you’re still seeing an issue here, please file a bug and provide a list of steps that can reproduce your problem. Please include a file that exhibits this behavior so that we can easily reproduce it on our test machines.
