This is all covered in detail in the LO documentation in Chapter 4 Forms. Basics about creation start on page 5 while adding fields is dealt with starting on page 48.

Well, the interface in my version (6.1.3.2) is different from that exposed in user’s guide, but finally I have found how to proceed. Thanks.
