I am very new to Databases and have created a very simple 1.
In brief I have 3 tables:
Employees
Phone Numbers
Devices
I want to be able to match the 3 together with rerecording information.
I would like to create a form where I will have a First Name and Last Name and based on these 2 bits of information it will know the key ID for that employee and enter it into a 4th table

How are the three existing tables related - Name, Employee ID, etc? First & last names are not unique. Have seen small companies with same names. Please edit your question with more information. Key ID? Unique? System generated?
