How to make a fillable pdf with checkboxes that shows if the input was correct ?

I would like to make a fillable pdf.

And I would like to give the user feedback on the input( Correct/Incorrect )

Also I want to use this in a Merge mail situation. So the check to see if the input is correct should take the answer fra a DataBase(SpreadSheet).
