shows one more black question mark

This weird glyph denotes a character not present in the font. To identify which character was typed (yes, this is indeed user error who typed some character to create desired spacing instead of using Math spacing directives):

  1. edit the formula (open it in Math)
  2. put the cursor at immediate right of spurious symbol
  3. press Alt+X
Alt may be another modifier key under MacOS

Report the “U+1234” result here.

If in a hurry, replace with a space, ~ (tilde) or " " (single or multiple spaces surrounded by double quotes.

@ajlittoz amply explained the subject matter. For future cases (also possibly useful with this one) please note:

A picture typically displays a symptom, but it does not always reveal the cause. For more to-the-point assistance you should attach the underlying element so we can more easily find the cause:

  • A document containing the offending object/issue
    … or …
  • The source for the issue,
    like …
    • Describe the procedure you performed
… or (more relevant in this case) …
    • Paste formula code copied from the bottom pane of the Math window

This is a file generated by others, I tried again, this file is a.doc suffix in word, clicking the formula will report errors, can not be edited, and found another .DOCx file, the formula can be edited normally, the formula shows up fine, it should be the word version of the problem, the word file showing error was created by word 2003 maybe, the office word i am using is 2016 version
thank you ! @ajlittoz @keme1

Maybe the Word error message when the formula is double-clicked is more useful

This was a problem with the WMF preview of the OLE objects. The MathType 6 formula uses “greater or equal” sign there; but it uses U+2026 symbol, and a specific font (Euclid Math Two, likely absent on your system). Notably, it is likewise wrong in Word and in Writer. MathType is full of wonders.

Here is the WMF to play with (renamed to .doc to please the site): image2.wmf.doc (1.5 KB)
