I create a table of contents up to Header 1 but Title entries are shown

1 English, Writer - how do I find out what version of Writer ?

I have created a poetry book. I have defined the title of all poems Header 1 the poems themselves Text Body and all notes (after each poem) Default. I have added a title & author name which are defined as Title.

When I create a table of contents up to Header 1 or using the default settings, the Title & author name show up as (separate) entries.

You need to apply a different style to the author name if you don’t want it to show up in the index. It should not use any of the Title or Heading styles. Then refresh your index.
