When I create an index, I see the index heading but no list
If you expect a useful answer, read these guidelines. Then, edit your question (don’t use an answer which is reserved for solutions) to be more descriptive.
Among other things, tell how you marked up the terms to be entered into the index.
Don’t forget to mention OS and LO index.
