Import Calc and Filter

Hello
I would like to import a Calc-Spreadsheet and do a Form that is able to Filter according to one colum.

Example:
Colum 1 - Value
Colum 2 - Item
Colum 3 - Subitem

The usecase should be that I have

1. Form
2. Filter with checkbox with colum 1
3. Table that shows colum 2+3 according to point 2

I just need a hint how to do that
