Problem with exporting .odt to .pdf

Hello,
I have created an odt file with background image, header and footer. When I exported it to pdf it is converted without them all. How can I solve this problem?

Libreoffice version: Version: 5.3.2.2. Mac Elcaptain 10.11.6

Lets try the the image first. What is the file format of your picture? EPS files will just make a blank.place holder. Also, just in case, use Insert > Image (choose embed if given the option) instead of copy/paste
