Attached is a PDF created with Libre office 6.3.1.2. When the docx file is converted to a PDF it does not bring over all the fields.
Kindly advise what is wrong.

No problem here with LO 7.4.3.2 under Fedora 37 (Linux).
General advice: always work with native application format. The native one is .odt and this protects you against cumulative conversion approximation when loading and saving your file.
